Bonjour à tous ! C’est Vanessa Montanari du blog de la recherche clinique. Tu as trouvé un SAE non déclaré depuis au moins quatre semaines sur le site, que faire ? Dans un premier temps, si le TEC est présent, le mieux c’est lui demander de compléter le SAE immédiatement pour envoi. Si l’investigateur est là, il faut également faire signer l’événement indésirable grave à l’investigateur. S’il n’est pas là, ce n’est pas un problème. Vous l’envoyez quand même, au service de PV immédiatement. Complétez même sans la signature du médecin. Si le TEC n’est pas là, si le médecin n’est pas là, tu dois prendre la décision d’envoyer le SAE complété. Tu complètes toi-même le SAE et tu l’envoies. De toutes les façons, il sera rectifié par la suite par le centre si jamais il y a une erreur ou un problème. L’étape suivante, c’est de reformer l’investigateur et le TEC, parce que s’ils ont laissé pendant quatre semaines un SAE sur le site non déclarée, il y a un problème. Donc, ils doivent être reformés et tu dois documenter cette formation sur un formulaire de formation pour bien monter en cas de souci par la suite que tu as bien formé le centre et qu’il était au courant, il a été formé sur l’avis d’initiation, il est reformé une deuxième fois. Dernière chose, tu documentes tout cela dans ton rapport de monitoring et dans ta lettre de suivi pour que l’investigateur soit clairement informé qu’il y a eu une déviation aux bonnes pratiques cliniques et qu’il doit faire attention pour les prochaines fois. You found an undeclared SAE at least four weeks ago on the site, what can you do?
First of all, if the CRC is present, the best thing to do is to ask her to complete the SAE immediately for submission. If the investigator is present, the investigator should also be asked to sign the serious adverse event. If the investigator is not present, this is not a problem. You send it anyway, to the VP department immediately. Complete even without the doctor’s signature. If the CRC is not there, if the doctor is not there, you have to make the decision to send the completed SAE. You complete the SAE yourself and send it. Either way, it will be corrected later by the centre if there is an error or problem.
The next step is to retrain the investigator and the CRC, because if they have left an SAE on the site for four weeks without reporting it, there is a problem. So they have to be re-trained and you have to document that training on a training form so that you can go up in case there’s a concern afterwards that you’ve trained the centre well and they were aware, they were trained on the initiation notice, they’re re-trained a second time.
Lastly, you document all this in your monitoring report and in your follow-up letter so that the investigator is clearly informed that there has been a deviation from good clinical practice and that he must be careful for the next few times.
